The vaccination team will be in school on Tuesday 18th January 2022 administering 2nd dose Covid-19 vaccinations to 12-15 year olds. A 1st dose will also be available to students who haven’t already been vaccinated. Please complete and return the consent form by Thursday 14th January 2021.
All completed forms should be returned to Nicola.flavell@nhs.net
